Enrolling in the Still Hopes Adult Day Program
Thank you for your interest in Still Hopes. We are delighted that you are considering making an application for residency or for the Adult Day Program in our memory assisted living community. The Rosalie W. Bomar Special Care Center specializes in assisted living for residents with a diagnosis of Alzheimer’s or other dementia related illnesses.
The application and admission procedure will be handled confidentially by Still Hopes. Applications are processed in chronological order according to the date they are received. To be considered for placement, a refundable deposit and a completed application (Personal Information Form) are required.

Health Requirements
Following an offer of admission, the prospective resident must provide Still Hopes with current medical information (Medical Certificate) from his/her physician. For admission to the memory assisted living community, the adult day program or respite care, a diagnosis of Alzheimer’s or related dementia disorder is required. This information must be submitted on forms which will be provided to you by Still Hopes and be received within thirty (30) days of move-in. After review of the medical documentation, Still Hopes will perform an in-home assessment to confirm appropriate placement. If your application is not approved for any reason, you will receive notification as soon as possible.
